Wayne County Health Dept Can Provide COVID-19 Vaccination Replacement Cards

The Wayne County Health Department can provide residents with a replacement COVID-19 vaccination card, if the original has been lost or destroyed.

A resident may request a replacement card by calling the Wayne County Health Department Clinic at 734-727-7100.

Our staff will verify the resident’s vaccination record through the state database and then issue a new card, which can be picked up at the Wayne County Public Health Clinic located at 33030 Van Born Road in Wayne, Mich.

If a resident has not yet received both doses of the Pfizer or Moderna vaccines, Wayne County can provide them with a copy of their vaccination record they can take to their second dose appointment. They will receive a new card at their second dose appointment.

Wayne County Health Department COVID-19 vaccination clinics are continuing to provide no-cost vaccinations to residents aged 12 and older. Residents can call 1-866-610-3885 to schedule an appointment or visit WayneCounty.com/COVID19 for more information about clinic locations and hours as well as walk-in vaccine opportunities.
