Dr. T. Jann Caison-Sorey, Medical Director, Clinical Quality and Network Management in Senior Health Services, Blue Cross Blue Shield of Michigan. She provides clinical leadership for the population based complex case management program focusing on the most prevalent conditions like Diabetes, Heart Failure, Chronic Lung Conditions, Asthma, and more. Dr. Caison-Sorey’s goal is to advance knowledge of these conditions and improve health outcomes by working collaboratively with physicians and integrating evidence-based medicine into clinical decision making.
Dr. Caison-Sorey remains active clinically with Henry Ford Health System and is a former Division Head, Pediatrics and Adolescent Medicine, and a founder of their School-Based and Community Health program. She is a Diplomat, American Board of Pediatrics; Fellow, American Academy of Pediatrics and Detroit Medical Academy; Past President, Wayne County Medical Society, Southeast Michigan and the Detroit Medical Academy; Clinical Adjunct Professor, Wayne State University School of Medicine; Former Adjunct Professor, University of Michigan, School of Public Health.
